Placement Research

After consultation, we conduct in-depth research on placement options in your area and provide a custom detailed report within 24 hours. We understand that time is of the essence. This research can cover services provided in outpatient, acute rehabilitation, rehabilitation, or skilled nursing to assisted living. We apply 25 years of experience to help you get an accurate picture of each placement option based upon your families needs and priorities. These are individual and custom reports unique to every client. Prices is determined through consulting but is included in the Short Term Consulting Package.

Placement Care Guides

This service is implemented after you have made your placement decision. The Placement Care Guides are customized and individually tailored by location and patient needs covering the first 5-7 days in the facility. They help you to know what to expect and have practical guidance on what you need to do during the entire process. These guides are the tools that enable you to be the Chief Influencer of Care (CIC), and will assist you in being the leader in your family’s decision. Price is determined through consultation but is included in the Short Term Consulting Package.

Short Term Consulting

Most clients have found that this consulting package helped them greatly not just in making the placement decision, but physically and emotionally, through the transition process. During and after transition, we provide coaching, consulting, and care guidance updates. This includes consulting regarding problems and coaching with you to ensure that you are heard by the health care professionals. This provides guidance through the normal procedures day to day in the facilities, and you will have 24/7 phone call access for questions, concerns, or emergencies. Families are encouraged to retain services for 60 to 90 day. This time frame has the greatest risk and is the most difficult time frame during the transition. Most past clients have found that within 90 days they have gained the skills and competence to navigate the process and have fully transitioned from crisis to “the new normal.” Pricing is determined during consulting based upon needs. The package is an all an inclusive package.

Long Term Consulting

We provide an on going consulting relationship for all families. When you run into a new barrier or cannot seem to get tractions on a problem… we are only a phone call away. Pricing is a flat hourly fee discussed during consultation.

Concierge Case Management

This service is provided on a by need basis. We provide longterm or short-term comprehensive case management services. Case Management is customized to supports you to the degree that your need or desire. It can include: On site visits, care review visits, temporary Power of Attorney with updates as few or many as you desire. Price is determined through consultation and can vary depending on the service needed.

Family Mediation

Unfortunately, there are times that families cannot reach consensus on care goals or placement needs. No matter the complicating factors involved, these decisions are ones that need and deserve consensus. We can help all parties down the road to ensuring that your family member’s needs are met. WE will and can help. Price is determined through consultation.
